that are actually happening in England, Wales, Northern Ireland and Canada.

information between health and social care, and how the South & East

staff and 120,000 patients.
In Vancouver an integrated hospital and community health management structure

Change
management and re-engineering the care plan in health and social
care
The South & East Belfast Health & Social Services
NHS Trust (SEBT) in Northern Ireland provide the full range
of primary, secondary and tertiary mental health services,
all community health services and social care programmes for
all groups of vulnerable people. These include mental health,
learning disability, physical disability, older people and
children's services.

Health
and social care integration in Swansea
Since starting its implementation of the in4tek Paris system
two years ago the City and County of Swansea social services
department has achieved considerable improvements in the delivery
of integrated care services. Swansea Social Services Paris
project manager, Tony Brindley, is now witnessing the evidence
of the changes that have taken place.

Torbay Council's social services are midway through their
implementation strategy to install the new Paris system that
will support integrated health and social care services and
assist the delivery of the single assessment process (SAP).
The first phase, which is already live, encompasses learning
disability services, with children's services and adult services
scheduled to go-live before the end of the year.

similar in structure to PCTs in England, about five years
ago.

welcome opportunity for joint working
Bristol City Council social services department, which covers
the largest unitary authority in England, serves a population
of over 400,000. With an annual budget of almost £121m,
the 3,000 plus staff work out of ten main offices and over
100 sites throughout the city, including eight hospitals,
day centres and childrens homes. Headquarters staff
and the youth offending team are based in another two offices.

Cardiff & Vale NHS Trust has announced that
it is rolling out an integrated electronic health record
for community nursing and mental health with the intention
of creating a fully integrated electronic health record.
